The resident could not see or access the button due to limited neck movement.
August 8, 2025: Failure To Follow Infection Control and Prevention Measures
Per the report, a certified nursing assistant entered an enhanced barrier precaution room and provided care without adequate protective gear, creating a risk of infection transmission.
May 5, 2025: Failure To Report Suspected Abuse To Proper Authorities
While reviewing a resident’s progress notes, inspectors learned that the resident’s responsible party reported that a certified nursing assistant acted in a “rough and rude” manner during the resident’s care and hurt her. The resident, who had a stroke affecting their non-dominant side, relied on staff for personal care.
The social services director said she interviewed the resident with the responsible party present, but the resident was confused and could not recall details. Staff recorded the complaint as a grievance, not suspected abuse. The director later acknowledged she is a mandated reporter and should have treated the report as potential abuse.
March 4, 2024: Failure To Maintain a Resident’s Fluid Intake
The facility was cited for failing to properly monitor and enforce a physician-ordered fluid restriction for a resident receiving renal dialysis. The doctor limited the resident to 1,200 mL of fluids per day. However, intake records show the resident exceeded that limit on at least 15 days within a one-month period.
The resident reported that staff never discussed the restriction and that a water pitcher remained in the room. A certified nursing assistant gave inconsistent statements, first denying knowledge of any restrictions, then admitting awareness but not knowing the allowed amount. Inspectors also found that the dietary director provided the water pitcher, and the restriction was not listed on the resident’s meal tickets.
