According to the report, staff did not check food temperatures before serving and did not keep a temperature log. When surveyors tested the food, several items were well below safe holding temperatures. Among them were cooked shrimp at 85 degrees Fahrenheit and chicken at 110 degrees Fahrenheit, well below the required 160 degrees for meat.
The inspection also revealed widespread sanitation concerns. Kitchen staff prepared food without hair or beard nets, and one employee handled food without washing their hands. Additionally, surveyors observed undated and unlabeled food items and uncovered plated meals stored in the refrigerator.
August 2022 Inspection Deficiencies
Another inspection in August 2022 found that Lake Park Healthcare Center failed to complete required admission and annual Minimum Data Set assessments for at least four residents, leaving key portions of their evaluations unfinished or completely blank.
