Throughout 2025, inspectors cited the facility for numerous deficiencies, including failing to follow treatment orders, failing to protect residents from abuse by other residents, failing to keep essential equipment in safe working condition, and failing to safeguard residents’ personal and medical records. Regulators also cited the facility for failing to provide safe and appropriate respiratory care when required, along with other violations related to resident health and safety.

That same year, the facility faced two fines totaling more than $60,000 in connection with abuse allegations, medication errors, and failures to notify physicians of changes in residents’ conditions. Medicare also imposed a payment denial in January 2025, stopping payments for new admissions until the facility corrected specific violations. The payment denial stemmed from a report in which one resident punched another resident in the head 10 times.

The Long Beach Post reports that regulators previously issued a rare “AA” citation to Long Beach Healthcare Center in 2019. This citation is reserved for cases in which a nursing home’s actions directly result in a resident’s death.
