A review of Skyline Healthcare Center LA’s inspection history shows an alarming pattern of serious violations related to neglect, abuse, safety, and poor quality of care. This facility has received $217,000 in fines and two payment suspensions from July 11, 2022, through April 1, 2025. Details about the most concerning violations are provided below.

May 2025: Resident-on-Resident Assault

During a complaint inspection, inspectors found that a resident punched another resident twice in the face, resulting in one-sided facial swelling and a broken, displaced nasal bone. A nurse was present during the attack. Inspectors found that staff failed to follow the aggressor’s care plan and document his aggressive tendencies or monitor mood swings and outbursts. Staff also did not document the incident properly.

April 2025: Unreported Abuse

While investigating a complaint, state surveyors determined that Skyline Healthcare Center failed to prevent another resident assault. According to the report, a resident with dementia and a documented history of aggressive behavior scratched her roommate’s foot with a remote control and left a 10-centimeter scratch that required first aid.

Just before the incident, the resident became agitated, yelled at staff, and swung the remote control at a CNA. Staff allowed the resident to walk past her roommate’s bed with the remote control while she was still agitated, rather than immediately removing her from the room.

The facility also failed to report the incident within the mandatory 24-hour timeframe. It received a $60,270 fine for the incident.

September 2024: Pain Medication Withheld and Controlled Substances Missing

A complaint inspection revealed that a resident reported pain with a severity of eight out of ten. The resident had a prescription for hydrocodone-acetaminophen, but staff only offered acetaminophen. This was inadequate to treat the resident’s pain.

Inspectors also discovered that the facility failed to lock the medicine cart drawer containing controlled substances. Inspectors found 20 opioid tablets missing and found residents to be in immediate jeopardy. The facility was fined $56,140.

December 2023: Preventable Fall Results in Fractured Femur

A complaint inspection revealed that a resident with a high fall risk suffered a fractured left femur and needed surgery after a CNA violated the resident’s care plan. The resident fell out of bed when the CNA turned him. The care plan required two-person assistance for bed mobility to prevent falls. The CNA admitted to being aware of the two-person requirement but claimed there were not enough staff available to assist.

June 2023: Failure to Report Sexual Abuse Allegation

While responding to a complaint, state inspectors found that a resident told an RN that a licensed vocational nurse inappropriately touched his buttocks. The RN failed to report it to the nursing home administrator, remove the accused nurse from care, or notify the California Department of Health. The administrator only became aware of the incident when the resident filed a formal grievance six days later.  

November 2022: Resident Fall Due To Lack of Supervision

According to a complaint inspection report, a resident with dementia fell out of bed after a CNA turned the resident without a second person to help and left them unattended. This resident required two-person assistance and extra fall prevention interventions due to being on blood thinners. The facility also failed to notify the physician immediately after the fall, as required by orders.

The resident suffered significant injuries but was not assessed until over 24 hours later. The injuries included a fractured clavicle, fractured rib, and multiple bruises. The resident reported a pain level of six out of ten and required an arm sling and pain medication for seven days.
