Sunray Healthcare Center is a skilled nursing facility in Los Angeles that offers subacute treatment, rehabilitation, and wound care services. Operated by Cambridge Healthcare Services, the 99-bed facility is located near Arlington Heights and averages 93 daily residents.

  • Over the past three years, more than 100 deficiencies have been documented at Sunray across three standard inspections and 42 complaint reports, including nine violations rated as presenting a risk of actual harm to residents.
  • The most recent severe violation was for failing to stop a resident who was on dialysis from escaping the facility in March 2024, despite knowing he had a history of drug abuse and was likely to attempt to leave.
  • Sunray Healthcare Center has been assigned by regulators to a federal program for facilities with “a history of serious quality issues” for nearly a full year.
  • Sunray has a 2.3-star rating on both Google and Yelp, with many reviews complaining of poor service and care quality.

Sunray Healthcare Center Facility Reputation & Ratings

Sunray Healthcare Center generally receives mixed to poor reviews on popular websites. The facility has received a 2.3-star rating on Yelp, based on 29 reviews, and the same rating on Google, based on 44 reviews.

On Yelp, negative reviews often cite short staffing and unprofessional, inattentive, or rude behavior by employees. Some claim their loved ones’ treatments weren’t followed through on or took too long to initiate, leaving them with suboptimal care. On Google, reviews ranged from praise for their family’s service quality to complaints of poor sanitation and maintenance, as well as allegations that loved ones were neglected or did not receive prescribed physical therapy.

Medicare.gov provides ratings for nursing homes based on staffing, quality measures, and health inspection results. However, no rating is currently available for Sunray Healthcare Center because of its “history of serious quality issues.” The website identifies Sunray as part of the Cambridge Healthcare Services chain, which has an overall rating of 2.5 stars.

Sunray Healthcare Center Identified as a Problem Facility

Sunray Healthcare Center has been named as a Special Focus Facility, or SFF. The federal Centers for Medicare & Medicaid Services oversees the SFF program, which aims to address systemic quality of care issues in nursing homes. According to the most recent list, which was published in September, Sunray has been named an SFF for 11 months.

States choose nursing homes for the SFF program based on the number, scope, and severity of deficiencies cited on recent health surveys. State agencies fully inspect chosen facilities twice a year and recommend progressive enforcement penalties for recurring issues, ending with the facility either graduating from the program or being excluded from participation in the Medicare and Medicaid programs.

Recent Complaints and Violations at Sunray Healthcare Center

Complaint and inspection reports collected by ProPublica show 126 total deficiencies found at Sunray Healthcare Center since August 2022, resulting in $126,000 in fines and two payment suspensions. Fifty-five of the violations were identified during standard inspections, while 71 were documented across 42 complaint reports.

Nine of the deficiencies were rated as “serious,” meaning they presented a risk of “actual harm” or “immediate jeopardy to resident health or safety.”

Sunray’s most recent CMS inspection report in April identified the following deficiencies:

  • Two violations related to nutrition and meal preparation
  • Failing to ensure that residents were properly receiving medication
  • Failing to properly document controlled substances
  • Failing to properly care for and prevent bedsores for at least one patient
  • Failing to ensure a call button was properly within reach of all residents
  • Failing to tell a resident that their Medicare benefits were ending
  • Failing to conduct a background check on at least three employees before hiring them
  • Having a medication error rate of 5% or higher

Former Staff Sued Sunray Healthcare Center for Overtime Pay Violations

In March 2024, Fernando Raymundo, a former respiratory therapist at Sunray Healthcare Center, filed a class action lawsuit against Sunray and Cambridge Healthcare Services for wage theft. Raymundo claimed the Sunray consistently paid him and other hourly workers at a standard rate for overtime work in violation of state and federal law.

This past February, a California federal judge ordered the parties to resolve the case through arbitration. As of October, the court had granted the parties’ joint request for additional time to discuss a potential settlement before beginning arbitration.

Widow Claimed Sunray Healthcare’s Negligence Led to Husband’s COVID-19 Death

In March 2021, Karen Segel filed a wrongful death lawsuit against Sunray Healthcare Center in state court. She alleged her husband, Marvin Bernard Almeas, died from exposure to COVID-19 as a resident at the facility. According to Segel’s complaint, Sunray failed to implement appropriate safety measures in response to California’s state of emergency during the emerging pandemic, including refusing to provide or allow staff to use masks.

Segel claimed Sunray management downplayed the pandemic and made no effort to isolate potentially exposed staff and residents. Almeas contracted COVID-19 at the facility and died in April 2020.

Sunray removed the case to federal court in September 2021, but the judge sided with Segel’s motion to remand it back to state court that December. Sunray appealed that order, but the 9th U.S. Circuit Court of Appeals affirmed the decision in June 2023.
