“There are five most common types of injuries that I see because of elder abuse and neglect. Number one is pressure sores. We see pressure sores all the time in nursing home cases, because the residents, especially the ones who are bed-bound, are not turned and repositioned or gotten out of bed enough, so they develop pressure sores commonly on their heels or their coccyx or backside. The number two type of injury we see from neglect in a nursing home is what we call the fall fracture case. This is when the elderly person falls in the facility because they have a high fall risk. Maybe they hit their call light and nobody comes to help them right away, they get up and try to go to the bathroom, they fall, and they have an injury.”

The Reality of Nursing Home Care Quality in Orange County
Orange County families trust nursing homes to provide quality care to their loved ones, but the unfortunate reality is that far too many facilities fall short of acceptable standards.
The latest information collected by the nonprofit investigative journalism organization ProPublica shows that every licensed nursing home in Orange County has been cited for at least one infection-related deficiency. Orange County nursing homes with histories of care issues include the following:
Advanced Rehab Center of Tustin
According to the data, this facility has received $37,182 in fines—the highest total among Orange County nursing homes. It was also cited for 123 deficiencies over the past three years, more than all but three other homes. Nine of the rehab center’s violations were infection-related, including the following identified in May:
- Failure to provide and implement an infection prevention and control program
- Failure to implement a program that monitors antibiotic use
- Failure to properly educate residents and staff about the COVID-19 vaccine, offer the vaccine, and accurately document vaccination statuses
Huntington Valley Healthcare Center
Huntington Valley reportedly has a sky-high nurse turnover rate of 58.8%—over 22 points higher than the state average and worse than any other Orange County home included in ProPublica’s data. The facility has been cited for 114 deficiencies in the past three years, including a $16,556 fine in February for violations, including failing to provide emergency care to a resident who later died.
Laguna Hills Health and Rehabilitation Center
According to ProPublica, Laguna Hills has been cited for 145 deficiencies since August 2023, the most of any Orange County nursing home during the covered period. Its most recent complaint report, issued in May 2026, identified the following violations:
- Failure to ensure that nurses and nurse aides are properly qualified to care for every resident’s needs
- Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing
- Failure to ensure each resident’s medication regimen is free from unnecessary drugs
Palm Terrace Healthcare & Rehabilitation Center
Palm Terrace reportedly received the second-most fines and had a particularly high turnover rate of 52.5%. According to ProPublica, the home was fined $33,118 and cited for 22 deficiencies in February, including one infection-related violation.
