Riverside PostAcute Care is currently on this list and has been for over a year.

Facilities on the list receive increased oversight from the Centers for Medicare & Medicaid Services and are inspected more frequently by state agencies. Since October 2022, Riverside PostAcute Care has faced more than $86,000 in federal fines. One of the most recent penalties, issued in March 2025, totaled $32,292. A 79-page inspection survey from the same date cited the facility for failing to dispose of garbage properly, not serving meals and snacks at appropriate times, understaffing its nursing department, failing to keep essential equipment in safe working order, failing to honor residents’ rights, among other concerns.

In that March 2025 survey, inspectors issued 52 health citations. That number is far higher than the state average of 16.2 and the national average of 9.8.
