  • Inland Valley Care and Rehabilitation Center has a two-star, below-average rating for staffing levels, with persistent complaints and citations for understaffing, including a citation for not having a licensed nurse on duty at all times.
  • This facility has been cited for administering psychotropic medications without consent. These types of medications are illegal chemical restraints.

Inland Valley Care and Rehabilitation Center’s Reputation

Inland Valley Care and Rehabilitation Center has a history of violations and complaints from residents and their families. State inspectors send inspection reports to the Centers for Medicare & Medicaid Services, which publishes the reports and the resulting ratings on Medicare.gov. Inland Valley Care and Rehabilitation Center has received an overall rating of one star, or “much below average.”

In addition, inspectors have found 169 health deficiencies at the facility during the last three years, according to ProPublica. These include 51 health citations in the previous 12 months alone.

According to US News & World Report, 8.8 percent of Inland Valley Care and Rehabilitation Center’s short-term residents have been hospitalized for serious infections, and its long-term residents had an average of 1.6 emergency room visits for every 1,000 patient days. The facility has paid $505,659 in fines during the last three years.

Inland Valley Care and Rehabilitation Center’s history of poor performance has landed it on the candidate list for the Centers for Medicare & Medicaid Services’ Special Focus Facility program. Special Focus Facilities receive heightened supervision and progressive enforcement until they consistently meet quality standards. This facility has been a candidate for 35 months.

Inland Valley Care and Rehabilitation Center Reviews

Inland Valley Care and Rehabilitation Center has a 2.4-star Google rating based on 83 reviews. While such reviews are not official documentation, they are consistent with state inspection findings. Reviewers report the following experiences:

  • Missing property, particularly cell phones
  • Difficulties reaching loved ones by telephone, especially after hours
  • Understaffing
  • Dirty conditions with offensive odors
  • Inattentive staff
  • Dressing residents who were incontinent in diapers because they required help getting to the restroom
  • Leaving residents in wet and soiled diapers for extensive periods
  • Unappetizing food
  • Malnourished residents
  • Difficulties visiting loved ones in hospice care

One reviewer alleges that facility staff “ripped out” her father’s catheter so harshly that he bled profusely and required an emergency room visit. According to the review, the facility delayed discharging him after the incident in an apparent attempt to conceal the incident. The reviewer also claims her father lost 35 pounds in three weeks due to not receiving the required assistance with feeding.

Underlying Causes of Inland Valley Care and Rehabilitation Center Violations

One of the most common causes of nursing home abuse and neglect is understaffing. Medicare has given Inland Valley Care and Rehabilitation Center a two-star rating for staffing. Understaffing often forces a single CNA or nurse to care for an unreasonable number of residents. It creates a high-pressure situation that may lead to the following forms of neglect and abuse:

  • Unanswered call lights
  • Leaving residents in soiled or wet diapers or clothing
  • Use of psychotropic drugs as chemical restraints
  • Inappropriate use of physical restraints
  • Use of medically unnecessary indwelling catheters
  • Failing to provide residents with showers or assistance with other daily activities
  • Medication errors
  • Abuse stemming from frustration and burnout

According to inspection reports, these violations have occurred multiple times. However, understaffing is only one potential cause of longstanding quality issues. Other potential causes include inadequate training, failure to perform background checks, and failing to budget for essential equipment and supplies, such as sanitation equipment and quality food for residents. The common thread is budgetary constraints, as for-profit nursing home owners often prioritize profits over residents.

Inland Valley Care and Rehabilitation Center Violations

The most recent health inspection on  August 30, 2024, resulted in 27 citations against Inland Valley Care and Rehabilitation Center, including the following:

  • Failure to keep residents’ call lights within reach
  • Failure to provide appropriate care to prevent bedsores
  • Failure to maintain or improve a resident’s range of motion
  • Failure to monitor a resident using a Foley catheter for urinary tract infections
  • Failure to elevate the head of a resident’s bed while using a feeding tube
  • Failure to provide necessary care to residents receiving oxygen
  • Failure to explore alternatives before installing bedrails on a resident’s bed
  • Failure to hire enough staff to have a licensed nurse on duty at all times and meet resident needs
  • Failure to safely administer medication as prescribed
  • Failure to obtain patient consent before prescribing psychotropic medication
  • A medication error rate of 21.43 percent—far above the five percent maximum
  • Failure to provide appetizing food that meets residents’ nutritional needs and accommodates allergies
  • Unsanitary kitchen practices during food preparation and storage
  • Staff performing work outside their qualifications
  • Failure to ensure the residents and their families understood arbitration agreements and inform them that they are optional
  • Failure to follow infection control guidelines, including hand sanitation

Violations Found During Complaint Inspections

Inland Valley Care and Rehabilitation Center had 13 complaint inspections from April 1, 2024, through March 31, 2025, which resulted in many of the same citations. Additional citations from complaints included the following:

  • Failure to include fall prevention in the care plan of a resident with a known fall risk, leading to a fall in the bathroom that resulted in an arm fracture
  • Failure to implement a scabies infection control program, with at least three residents infected
  • Failure to hire enough staff to meet residents’ needs, including showers, assistance with transfers from beds to wheelchairs, and timely adult brief changes
  • Failure to report suspected abuse when a resident told a staff member that a CNA was physically hurting her, using the excuse that the resident displayed no physical signs of abuse
  • Failure to protect a resident from physical abuse by another resident with known violent tendencies
