It has a history of low ratings, health and safety violations, and substantiated abuse allegations. All Saints Healthcare Subacute Ratings and Reviews
All Saints Healthcare Subacute has a low Google review rating of 3.2 stars, with some reviewers alleging that their loved ones suffered abuse and severe neglect. One reviewer noted that their loved one suffered bruises from rough handling. Others stated that staff are slow to respond, lack empathy, and are rude to concerned family members.
Medicare.gov gave All Saints Healthcare Subacute an above-average rating for quality measures. However, this does not necessarily mean the facility is safe or that your loved one would receive quality care there. The Centers for Medicare & Medicaid Services reports the following red flags in the quality of care for long-term care residents:
- 8.3% of residents have bed sores, compared to 5.4% nationally and 5.1% statewide.
- 7.4% of residents have indwelling catheters compared to a state and national average of 1.2%.
- 4.7% of residents have been physically restrained compared to 0.1% nationally and 0.4% statewide.
- 34.9% of residents received anti-anxiety or hypnotic medications compared to 19.9% nationally and 13.7% statewide.
All Saints Healthcare Subacute Violations and Complaints
All Saints Healthcare has an overall rating of one star, or “much below average,” on Medicare.gov due to its poor performance during state inspections and its history of substantiated complaints. All Saints Healthcare Subacute had 97 deficiencies in the last three years, including 33 in the last year alone. The average nursing home in California had 16.1 health citations during the same period, and the national average was 9.5. During the last three years, federal All Saints Healthcare Subacute has been fined $154.996 by federal regulators.
Nursing homes typically undergo one unannounced health inspection annually, as well as additional inspections in response to complaints. 2024 Health Inspection Deficiencies
Several of the deficiencies inspectors found during the health inspection on October 25, 2024, directly impacted resident safety and quality of care. They include the following:
- Failure to ensure call lights were within all residents’ reach
- Inappropriate use of physical restraints
- Failure to provide appropriate bed sore care
- Improper catheter care
- Failure to maintain feeding tubes to sanitary standards
- Failure to provide appropriate respiratory care for residents as required
2023 Health Inspection Deficiencies
Inspectors also identified several concerning deficiencies during the health inspection on October 20, 2023. Many are similar to the deficiencies found in 2024, indicating a continuing pattern of negligence. These deficiencies include the following:
- Failure to ensure call lights were within all residents’ reach
- Multiple medication errors, including incorrect dosages, administering incompatible medications, inappropriate administration techniques, and failing to administer complete dosages
- Failure to provide appropriate bed sore care for eight residents
- Failure to use proper catheter care for four residents and use a dignity bag for two others
- Failure to ensure that feeding tubes were only used when medically necessary, and to use safe feeding tube practices
- Failure to provide respiratory care to professional standards
- Failure to train staff to meet residents’ specific needs
Complaint Inspections
The California Department of Health investigates complaints about nursing home abuse, neglect, and quality of care concerns and often performs inspections in response.
From September 1, 2024, through August 31, 2025, this facility had 24 complaints that resulted in citations, some of which were related to abuse and neglect. The inspections uncovered the following:
- May 15, 2025: Failure to timely report suspected abuse of a resident with dementia who had a gastronomy tube and tracheostomy. A CNA reportedly yelled at the resident to “Open, open, open” during assisted feeding.
- May 2, 2025: Failure to timely report suspected abuse of a resident who reported a sexual assault by a nursing home employee during a shower the previous year
- April 29, 2025: Failure to notify a physician or family member when a resident developed tachycardia and a low-grade fever
- December 12, 2024: Failure to report or investigate abuse of a resident who reported being struck on the jaw twice by staff
- June 19, 2024: Failure to immediately investigate or report unexplained bruising on a cognitively impaired resident’s jaw, which may have arisen from an assault
- June 19, 2024: Infection control violation by a CNA who administered medication through a resident’s gastronomy tube without a protective gown
