Preventable Pressure Injury at Bay View Rehabilitation Hospital
In a September 2024 inspection, surveyors cited Bay View Rehabilitation Hospital for failing to prevent a pressure injury caused by a medical device. The resident had quadriplegia and depended entirely on staff for repositioning. He developed a pressure injury on the left upper back after lying on a nephrostomy tube.
According to the inspection report, facility records showed that staff identified a wound measuring 3.3 cm by 1.3 cm, and a nurse later acknowledged that the injury could have been prevented by making sure the resident was not positioned on the tube. However, the resident’s care plan still did not include instructions to prevent the resident from lying on the nephrostomy tube.
Infection Control Concerns for Residents With Pressure Ulcers at Alameda Healthcare & Wellness Center
An August 2024 inspection of Alameda Healthcare & Wellness Center resulted in citations for multiple infection-control failures involving residents subject to enhanced barrier precautions. The inspection report said that according to the facility’s policies, these precautions apply to residents with chronic wounds, including serious pressure ulcers. Despite these policies, surveyors observed staff members failing to follow proper hand hygiene and personal protective equipment protocols while caring for high-risk residents.
Because pressure ulcers can become infected, proper infection control practices are particularly important for those residents. While inspectors didn’t cite the facility for directly causing or worsening pressure ulcers, those failures could increase the risk of infection for residents with significant skin wounds.
Multiple Residents Identified With Pressure Ulcers at All Saints Subacute & Transitional Care
In a March 2025 inspection of All Saints Subacute & Transitional Care, several residents were identified as having serious pressure injuries:
- A resident had developed a pressure ulcer in the sacral region.
- A resident had stage 4 pressure injuries—the most advanced stage of bedsores—on both hips.
- Another resident also had a stage 4 pressure ulcer of the sacral region.
- A resident was diagnosed with deep tissue damage in the sacral area.
- A resident had unstageable pressure injuries to the right heel and sacral region.
