Serious Pressure Ulcer Injuries at Reo Vista Healthcare Center

According to a July 2021 report, Reo Vista Healthcare Center was cited for staff’s failure to properly program a pressure-relieving mattress for a resident with a Stage 3 pressure ulcer on the lower back. The resident weighed 85 pounds, but the mattress was set for 180 pounds on July 19, 2021, and later changed to 120 pounds. The report stated this created a risk of worsening the resident’s condition and increasing complications.

Pressure Ulcer Assessment and Treatment Failures at Bayshire Torrey Pines Post Acute

A February 2025 report found that Bayshire Torrey Pines Post Acute failed to properly assess and treat a resident’s pressure ulcer and failed to take appropriate steps to prevent additional pressure ulcers. The report stated that staff did not correctly identify the resident’s existing pressure ulcer during the admission assessment or provide care that met accepted standards. As a result, the resident’s Stage III pressure ulcer was not properly staged, delaying the treatment needed to help prevent further progression.

A May 2025 report found that University Care Center did not consistently follow pressure ulcer prevention protocols for a resident. The report stated that staff failed to reposition the resident every two hours.
