  • Failure to provide appropriate care for a stage four bedsore that a resident had developed at the facility
  • Failure to ensure that three residents were free of fall hazards and complete all required neurological monitoring for one resident following a fall
  • Failure to administer oxygen therapy according to the doctors’ orders
  • Stored oxygen therapy equipment improperly
  • Gave at least one resident continuous oxygen without a doctor’s order
  • A medication error rate of 7.41 percent, in violation of the five percent maximum
  • Unsanitary conditions in food preparation areas

During a November 2024 complaint inspection, inspectors found that multiple staff were present during the removal of a female resident’s urinary catheter, including two male staff, without the resident’s permission. The resident told inspectors that she felt violated and cried afterwards. Only one nurse is required to remove a catheter.

Huntington Valley Healthcare Center

Huntington Valley Healthcare Center has had 48 citations in the last year and 32 complaints that resulted in citations during the last three years. The most serious deficiencies inspectors uncovered at Huntington Valley Healthcare Center include the following:

  • Failure to report or investigate after receiving a complaint from a resident who said her roommate verbally abused and threatened her all night
  • Failure to accommodate a resident’s food allergies, causing him an allergic reaction that required hospitalization 
  • Unsanitary conditions in a shared resident’s restroom, including used washcloths by the sink, an unlabeled pitcher, and a soiled laundry collection bin pushed by a bare-handed staff member
  • Failure to perform laboratory testing ordered by a physician or complete required neurological checks on a resident who had fallen
  • Unnecessary and improper psychotropic medications administered to dementia patients

Sea Cliff Healthcare Center

Sea Cliff Healthcare Center has the red hand icon next to its name on Medicare.gov, indicating it was recently cited for abuse. It has been cited for 19 deficiencies during the last year and 28 substantiated complaints during the last three years. A September 6, 2024, complaint inspection revealed that two residents were injured in a physical altercation when a resident scratched another resident, who responded by biting their hand.

  • A 20 percent cumulative medication error rate, violating the medical standard of no more than five percent
