Diamond Ridge Healthcare Center

In August 2023, Diamond Ridge Healthcare Center received a $42,033 fine after inspectors found excessive indoor temperatures exceeding 81 degrees Fahrenheit. Due to the facility’s failure to maintain its cooling system, residents experienced physical discomfort, nausea, sleep difficulties, and disruptions to restorative nursing care. Inspectors determined that the conditions posed a risk of heat exhaustion or heat stroke for multiple dependent, bed-bound residents.

San Pablo Healthcare & Wellness Center

In February 2024, Medicare fined San Pablo Healthcare & Wellness Center $10,036. Inspectors found that the facility failed to properly supervise a resident who smoked and allowed the resident to possess cigarettes and lighters. During the same inspection, the facility was also cited for allowing residents to self-administer medications without the required physician orders and safety assessments.

Willow Pass Healthcare Center

In April 2024, Willow Pass Healthcare Center was fined $45,702 after inspectors found that the facility failed to protect a resident from verbal and physical abuse by another resident. An altercation resulted in a skin tear and left the resident fearful for their safety.
